PACE, (Program of All-inclusive Care for the Elderly), is a comprehensive health care program for seniors who have disabilities or health concerns and wish to remain in their home while receiving all needed medical and supportive services. PACE offers an entire continuum of care from medical services to medications, from nutritious meals to transportation – all personalized to meet your specific needs.
Senior TLC is proud to offer PACE services to our community. This model of health care has been in existence for over 40 years throughout the United States. The program focus is on prevention, wellness, and a commitment to enhance the quality of life of seniors and their families through care and support.
