A referral can also be made by a family member, physician or other provider who is familiar with your condition. To make a referral call 704-874-0600. To enroll at Senior TLC you must meet the following requirements:

• 55 years of age or older.

• Must meet skilled nursing level of care.​

• Live safely in the community at the time of enrollment with Senior TLC services.

• Must live within our service area.
