You can actually refer yourself!

A referral can also be made by a family member, physician or other provider who is familiar with your condition.

For medical offices and other professionals (social services, home health agencies, etc.), download the referral form below, complete and fax to 704-834-1998. To enroll at Senior TLC you must meet the following requirements:

• 55 years of age or older.

• Must meet skilled nursing level of care.​

• Live safely in the community at the time of enrollment with Senior TLC services.

• Must live within our service area.
