Interdisciplinary Team
The Interdisciplinary Team (IDT) is made up of doctors, nurses, therapists, social workers, activities, dietitian, home care coordinators, transportation coordinator, nursing assistants and center manager. The IDT focuses on each individual participant to perform assessments and create a plan of care.
